What attracted you to apply for Gaia Sagrada Community living? What do you hope to gain? Why would you like to live here?*

Please describe your financial situation. Do you work online? If so how many hours per day? What type of work? Do you have a savings or some other kind of income not related to work?*

Internet in the community center area only, not the rooms, so your work would need to be quiet and not a lot of talking online, like doing counseling sessions and such. That sort of work would have to be done in your room, but it wouldn't be fair if you have a roommate. Cell phone internet is not dependable, but international satellite internet might work better. Check with your phone carrier if it works here in Ecuador. There would be nowhere in the community center you could do session type work as there are no private spaces in the community center.

How do you feel about vaccination?*

Gaia Sagrada will offer retreats for as long as possible, which will include vaccinated and unvaccinated people mixed. Please let us know if you are ok with this and will take precautions/supplements if you feel they are needed. During times when there are no retreats, only unvaccinated will be here.

Have you ever or do you now use alcohol or drugs like marijuana, cocaine, methaphetamines, heroin or other drugs in an addictive or social fashion? Please explain your feelings about this.*

Do you have any medical conditions like diabetes, high blood pressure, heart, respiratory, or any other physical issues?*

If you use medications for these conditions, make sure you bring plenty of your medications. Bring your prescription as well so you can have them filled in Ecuador.

How physically fit are you? Injuries? Tell us about your physical condition.*

Keep in mind that the grounds are uneven in some places. It would be impossible to get around in a wheelchair. You will need to be able to walk up hills on uneven ground to get around.

What are your spiritual and religious interests and leanings? Are you at ease with people who have other spiritual paths? Are you at ease with the fact that Gaia Sagrada conducts shamanic Ayahuasca and San Pedro medicine ceremonies?*

MEDICAL AGREEMENT*

I understand that Gaia Sagrada cannot provide me with medical attention nor does Gaia Sagrada have a doctor or nurse on site. I understand that the hospital is approximately an hour away. I am alerting Gaia Sagrada in the "special dietary or medical needs" section if I have health conditions which staff needs to know about for my safety. By submitting my application for community living, or any other type of stay at Gaia Sagrada, I understand that my health and well-being is my responsibility and not that of Gaia Sagrada, staff, or associates thereof. I am bringing medicines I need with me, I have consulted with my doctor if I feel the need, and I have enough money to take care of my own health needs. If I have signed up for the shamanic retreat, or choose to when I am there, I take full responsibility for my experiences or reactions to the Sacred Ceremonies. I choose to participate in Sacred Ceremonies, or simply stay at Gaia Sagrada without being a participant in the retreats, of my own free will. If participating in the shamanic ceremonies, I know that I will be partaking in plant medicine teas. Whatever type of stay at Gaia Sagrada I choose, I promise to be careful, follow directions, and ask for help if I should need assistance in any way whatsoever.

COVID 19 RISK*

I, of sound mind, understand that there is an ongoing global COVID-19 pandemic and that traveling to Gaia Sagrada and to Ecuador, for a retreat or a monthly stay, is not risk free. I take total responsibility for my own health and any medical expenses that may arise due to my decision to travel. I agree that if I were to fall ill during my time at Gaia Sagrada with any cold or flu-like symptoms, I will immediately enter into isolation. Refusal to isolate will result in my immediate dismissal from Gaia Sagrada. I agree that if I were to become sick with COVID-19, or any other virus, I will not hold Gaia Sagrada responsible. I acknowledge that social distancing and facemasks, etc are not practiced at Gaia Sagrada and that Gaia Sagrada cannot guarantee a virus free zone. I agree to take full responsibility for my decision to attend a retreat or monthly stay at Gaia Sagrada. I also agree to practice safety when visiting town, wear a mask in public places as required by Ecuador and not go against Ecuador's requirements for public practices with social distancing and masks when I am not on the Gaia Sagrada grounds. I agree to obey Ecuador's laws when in public places outside of Gaia Sagrada since I am a guest in Ecuador. I understand Ecuador is not the place to rebel against pandemic rules so as not to attract attention to Gaia Sagrada.
